One of the most basic things to understand about an ePub file and an ePub reader is that an ePub file has a standard viewing format encoded into it with HTML. Publishers release books intending for them to look a certain way. Thus, they establish text placement, title font size (relative to body text), font colors, and even includes the font files themselves hardcoded into the books. An ebook reader has to go out of its way to change this. MarginNote should allow for the book to be rendered in its default presentation.
